  • Pathology reports
  • Medicare Number

We understand that health information is highly personal and that your privacy be respected.

Use of Personal Information

Information that you provide at consultation may be shared with another health service provider, for example your referring doctor. This is in accordance with professional codes of practice, to deliver optimum health care.
